Periodontal (gum) disease can do unpleasant things to your mouth, including losing teeth. Its effects, though, may not be isolated to the oral cavity: Gum disease could make other diseases in the body worse.
Gum disease is a bacterial infection most often caused by dental plaque, a thin bacterial film that builds up on teeth in the absence of effective oral hygiene. At the outset it may infect your gums causing them to swell, redden or bleed. Eventually, though, the infection can advance deeper toward the tooth roots and bone.
There are various methods to treat gum disease depending on the extensiveness of the infection. But these methods all share the same objective—to remove all uncovered plaque and tartar (hardened plaque). Plaque fuels the infection, so removing it starves out the disease and helps the body to heal.
The damage gum disease can do to the teeth and the surrounding gums is reason enough to seek treatment. But treating it can also benefit your overall health. That's because the weakened gum tissues often serve as an open portal for bacteria and other toxins to enter the bloodstream. From there they can travel to other parts of the body and cause disease.
Gum disease also shares another feature with some systemic conditions: inflammation. This is the body's response to disease or trauma that isolates damaged tissues from healthy ones. But with gum disease, this inflammation can become chronic and ironically do more harm than good.
A gum infection may also increase the body's overall inflammatory response, in turn aggravating other diseases like diabetes, heart disease or arthritis. Treating gum disease lowers inflammation, which in turn could ease inflammation in other conditions. Likewise, reducing your body's overall inflammatory response by properly managing these other conditions might make you less susceptible to gum disease.
It's important then to prevent and treat gum disease as if your overall health depended on it—because it does. You can prevent it by brushing and flossing daily and undergoing regular dental cleanings to remove plaque. And see your dentist promptly at the first signs of gum problems. Likewise, follow a physician-supervised program to manage any inflammatory conditions.

Periodontal (gum) disease often involves more than gum inflammation. The real danger is what this bacterial infection may be doing to tissues beneath the gum line—including tooth roots and supporting bone.
Gum disease can do extensive damage to the forked areas where the roots separate from the main tooth body. If one of these areas, known as a furcation, becomes infected, the associated bone may begin to diminish. And you may not even know it's happening.
Fortunately, we may be able to detect a furcation involvement using x-rays and tactile (touch) probing. The findings from our examination will not only verify a furcation involvement exists, but also how extensive it is according to a formal classification system that dentists use for planning further treatment.
A Class I involvement under this system signifies the beginning of bone loss, usually a slight groove in the bone. Class II signifies two or more millimeters of bone loss. Class III, also called a “through and through,” represents bone loss that extends from one side of the root to the other.
The class of involvement will guide how we treat it. Obviously, the lower the class, the less extensive that treatment will be. That's why regular dental checkups or appointments at the first sign of gum problems are a must.
The first-line treatment for furcation involvements is much the same as for gum disease in general: We manually remove bacterial plaque, the main source of infection, from the root surfaces using hand instruments and ultrasonic equipment. This is often followed by localized antibiotics to further disinfect the area and stymie the further growth of the furcation involvement.
We also want to foster the regrowth of lost tissue, if at all possible. Classes II and III involvements may present a challenge in this regard, ultimately requiring grafting surgery to stimulate tissue regeneration.
The best approach by far is to prevent gum disease, the ultimate cause for a furcation involvement. You can reduce your chances of gum disease by brushing and flossing daily to remove disease-causing plaque. Regular dental cleanings and checkups, at least every six months, help round out this prevention strategy.
A furcation involvement could ultimately endanger a tooth's survival. We can stop that from happening—but we'll have to act promptly to achieve the best results.

How do you know if you have periodontal (gum) disease? Sometimes your gums will tell you—when they’re red, swollen or bleed easily.
But your gums can also look and feel healthy while a gum infection still brews below the gum line. In this case, a regular dental visit could make the difference. Even without overt signs of infection, we may be able to detect gum disease with a slender metal instrument called a periodontal probe.
Gum disease is a bacterial infection that most of the time arises from dental plaque. This thin film of bacteria and food particles accumulates on tooth surfaces, especially because of poor or non-existent oral hygiene. A continuing infection can weaken gum tissues and cause them to pull away or detach from the teeth.
Normally, there’s a slight gap between the gums and teeth. But as the infected gums pull away, the gaps grow larger and deeper, forming what are known as periodontal pockets. They become filled with infection that soon spreads to the root and bone and increases the risk of tooth loss.
These pockets, though, could be the means for detecting a gum infection with the help of the periodontal probe. During a dental exam we gently insert the probe, which has millimeter depth markings etched on it, between a tooth and its adjacent gums. While a depth of 1 to 3 mm is normal, a probe measurement of 4 to 5 mm could be a sign of an early stage infection. A reading of 7 to 10 mm, on the other hand, may indicate more advanced disease.
Along with other factors, periodontal probing can be quite useful identifying both the presence and extent of a gum infection and then how to treat it. The goal of any treatment is to remove plaque and tartar (calculus) deposits that sustain the infection. But probing, along with other diagnostic methods like x-rays, could point to deeper infection below the gum line that require more extensive methods, including surgery, sometimes to access and remove the disease.
Achieving the best treatment outcome with gum disease often depends on finding the infection early. Periodontal probing helps to make that discovery more likely.
Here’s the bad news about periodontal (gum) disease: It’s a leading cause for tooth loss. Even worse: Half of adults over 30 will have some form of it during their lifetime.
But here’s the good news: If caught early, we can often treat and stop gum disease before it can do substantial harm to your mouth. And the best news of all—you may be able to avoid a gum infection altogether by adopting a few healthy habits.
Here are 4 habits you can practice to prevent a gum infection from happening.
Practice daily brushing and flossing. Gum disease is a bacterial infection most often arising from dental plaque, a thin film of bacteria and food particles that accumulates on teeth. Removing plaque daily with brushing and flossing will reduce your chances of a gum infection. And be sure it’s daily—missing just a few days is enough for gum inflammation to get started.
Get regular dental cleanings and checkups. Even the most diligent personal hygiene can miss plaque, which may then harden into a calcified form impossible to remove with brushing and flossing called calculus (tartar). At least twice-a-year professional dental cleanings will clear away any remnant plaque and tartar, which can greatly reduce your risk for dental disease.
Make gum-friendly lifestyle changes. Smoking more than doubles your chances of gum disease. Likewise, a sugar-heavy diet, which feeds disease-causing bacteria, also makes you more susceptible to infection. Quitting smoking, cutting back on alcohol consumption and following a dental-friendly diet could boost your teeth and gum health and avoid infection.
Watch for signs of infection. Although you can greatly reduce your risk of gum disease, you can’t always bring that risk to zero. So, be aware of the signs of gum disease: sometimes painful, swollen, reddened or bleeding gums. If you notice any of these signs, make a dental appointment—the sooner you’re diagnosed and begin treatment, the less likely gum disease will ruin your dental health.
Although periodontal (gum) disease starts with the gums, the teeth may ultimately suffer. An infection can damage the gum attachment and supporting bone to the point that an affected tooth could be lost.
The main cause for gum disease is dental plaque, a bacterial biofilm that accumulates on teeth due to ineffective oral hygiene. But there can be other contributing factors that make you more susceptible to an infection. Smoking tobacco is one of the most harmful as more than half of smokers develop gum disease at some point in their life. If you’re a heavy smoker, you have double the risk of gum disease than a non-smoker.
There are several reasons why smoking increases the risk of gum disease. For one, smoking reduces the body’s production of antibodies. This diminishes the body’s ability to fight oral infections and aid healing. As a smoker, your body can’t respond adequately enough to the rapid spread of a gum infection.
Another reason for the increased risk with smoking are the chemicals in tobacco that damage the connectivity of gum tissues to teeth that keep them anchored in place. The heavier the smoking habit, the worse this particular damage is to the gums. This can accelerate the disease and make it more likely you’ll lose affected teeth.
Smoking can also interfere with getting a prompt diagnosis of gum disease because the nicotine in tobacco reduces the blood supply to the gums. Usually a person with an infection may first notice their gums are reddened or swollen, and bleed easily. Smoking, however, can give a false impression of health because it prevents the infected gum tissues from becoming swollen and are less likely to bleed. As a result, you may learn you have the disease much later rather than sooner, allowing the infection to inflict more damage.
There are ways to reduce your disease risk if you smoke. The top way: Kick the smoking habit. With time, the effects of smoking on your mouth and body will diminish, and you’ll be better able to fight infection.
You should also practice daily brushing and flossing to keep plaque at bay, followed by regular dental cleanings to remove hard to reach plaque and calculus (tartar) deposits. You should also see your dentist at the first sign of trouble with your gums.
